§ 12-08 Additional Provisions.

RCNY § 12-08

(a)For so long as the Program uses a card to facilitate the discount for some types of transportation, the Program reserves the right to limit the number of replacement cards it will issue to an individual within an enrollment year, in accordance with the Conditions of Use pursuant to DSS/HRA's Memorandum of Understanding with NYCT.

(b)The Program reserves the right to require anyone who has been issued a card to return their card that has been reported as damaged or that has been deactivated or has expired. The Program further reserves the right to impose reasonable restrictions, in accordance with the Conditions of Use, on the ability of individuals who have failed to return a card at the request of the program to receive a new card.

(c)Individuals who have been permanently disqualified from the Program under 68 RCNY § 12-06(b) may petition the Program for reinstatement after one year and may be reinstated if they can demonstrate extraordinary circumstances justifying reinstatement.
